Add Clinical Logs

Students use Clinical Logs to document clinical experiences—encounters, skills, times, and notes—then submit them for preceptor review.

Overview

Open Clinical Logs from the main navigation and choose Add Clinical Log. Your classroom’s clinical log template drives the fields on each encounter. Save drafts anytime; submit when the log is ready for review.

Before you submit

You need:

  • Classroom and preceptor
  • Date, start time, and end time (total hours calculate automatically)
  • At least one patient encounter

If location verification is enabled for the classroom, use the key control next to start/end time and enter the code your instructor shares. Verified times lock in and the log may auto-save as a draft.

Encounters

  1. Click Add Encounter for each patient or case you document in the shift.
  2. Complete the template fields (text, selects, ICD-10/CPT search fields, and so on—whatever your program configured).
  3. Add skills for that encounter: choose template skills and record counts for Performed, Assisted, and/or Observed. You can add a custom skill if needed (ask your program whether custom skills count toward requirements).
  4. Add notes as needed. Remove an encounter with the trash control if you added one by mistake.

AI Scribe (when available)

If student AI is enabled for your classroom, use AI Scribe on an encounter:

  1. Open the microphone control and allow browser access if prompted.
  2. Speak your notes, then stop recording.
  3. Review what was filled in—demographics, skills, codes, and summary text—and correct anything before you submit.

You can also generate a SOAP-style note from a short summary when that helper is shown. Always review AI output for accuracy and never include patient identifiers.

Drafts and submit

  • Save as Draft keeps your work without sending it for review.
  • Submit Log sends the log to your preceptor (status becomes pending). Fix any validation messages first.
  • If a log is rejected, open it from Clinical Logs, address the feedback, and resubmit.
  • Approved logs are locked; contact your preceptor if something must change.

Logs started from a completed shift may arrive partially filled—confirm times, skills, and encounters before submitting.

Tips

  1. Document soon after the shift while details are clear.
  2. Use one encounter per patient/case for cleaner skill tracking.
  3. Double-check times and verification before submit.
  4. Keep notes free of names, MRNs, and other identifiers.

Troubleshooting

AI Scribe missing
Your classroom or institution may not have student AI enabled, or microphone permission was denied.

Skills list empty
Select a classroom that has a skills template assigned, or ask an admin to attach one.

Cannot submit
Fill required header fields, add at least one encounter, and enter skill counts where the form requires them.

Verification code fails
Ask your instructor for the current classroom code; it may have been reset.
