Evaluation Templates
Evaluation templates are the forms used to assess students, preceptors, or clinical locations. Admins build them under Templates → Evaluations.
Overview
Open Templates → Evaluations, create or edit a template, then assign it from Create Assignments. Completions appear under Assignments (see My Evaluations help).
Target types
Choose who the evaluation is about:

The target type shapes assignment pickers and how questions should be worded.
Create a template
- Go to Templates → Evaluations.
- Create a new template (or duplicate an existing one).
- Set name, target type, grading type, and score type (pass/fail or percentage).
- Optionally attach a Competency Map template so questions can map to competencies.
- Add questions and section headers, then save.
Grading
- Satisfactory / Unsatisfactory — binary scoring
- Likert scale — multi-point ratings
- Custom scoring — institution-defined labels and values via a rubric you create or select
Pick the grading style that matches how your program scores this assessment, and keep it consistent across similar templates.
Questions
Add items as questions (scored) or headers (section labels only).
- Write clear, observable statements (for example “Demonstrates effective handoff communication”).
- Use headers to group long forms.
- Reorder items when the editor supports drag-and-drop.
- Import questions from a spreadsheet when you already have a bank of items.
Optional AI-assisted template generation or curriculum mapping may be available when educator AI is enabled for your institution. Treat suggestions as a draft—review before you rely on them for accreditation.
Curriculum mapping
If you selected a Competency Map template:
- Open mapping on a question.
- Search and select the competencies or subcompetencies it addresses.
- Prefer a few direct alignments over mapping everything to everything.
Mapped evaluations feed curriculum coverage views and reporting over time.
Tips
- Name templates by purpose and target (for example “Mid-rotation student eval”).
- Keep forms focused (roughly 15–30 scored items) so evaluators finish them.
- Stabilize your Competency Map before large mapping projects.
- Duplicate a working template when you need a close variant for another rotation.
Troubleshooting
Template not available when assigning
Confirm it is active (not archived) and matches the target type you are assigning.
Cannot map to competencies
Select a Competency Map template on the evaluation template, and confirm that map has competencies defined.
Import failed
Use the downloadable spreadsheet sample, keep a question-text column, and re-upload.
