Clinical Log Templates

Clinical log templates define the fields students complete when they record clinical experiences. Admins author them under Templates → Clinical Logs.

Overview

Open Templates, select the Clinical Logs tab, then create or edit a template. Assign the finished template to a classroom so students see those fields when adding logs.

Create a template

  1. Go to Templates → Clinical Logs.
  2. Create a new template (or open an existing one).
  3. Give it a clear name (for example by profession, rotation, or program).
  4. Add fields, or start from a profession preset when offered (nursing, medical, PA, therapy, and similar).
  5. Save and return to the list when you are done.

Presets are a starting point—you can add, remove, reorder, and edit fields afterward. Applying a preset over existing fields typically replaces them; confirm before you continue.

Fields

Each field has a label students see, a type, and optional required flag.

Common types:

You can also add common standard fields (demographics, setting, coding, acuity, and similar) in bulk from the add-field flow.

Field tips

  • Put fields in the order students encounter information on site.
  • Mark only essential fields as required.
  • Keep select options short and consistent across similar templates.
  • Drag fields to reorder when the editor supports it.

Assign to a classroom

  1. Open Classrooms and edit the classroom.
  2. Choose the clinical log template for that classroom.
  3. Save.

Students in that classroom then use the template when creating clinical logs. Existing logs keep the data already recorded; changing the template affects new entries going forward.

Tips

  1. Start from a preset when it matches your profession, then trim unused fields.
  2. Use one template per classroom or rotation type so reports stay comparable.
  3. Test with a sample log before you roll a new template out to a large cohort.
  4. Archive old templates instead of deleting when historical logs still reference them.

Troubleshooting

Students do not see the expected fields
Confirm the classroom has this template selected, students are in that classroom, and they refresh after the change.

Template will not save
Enter a template name and confirm you are signed in as an institution administrator.

Select options look wrong
Edit the field’s option list, leave the field so changes save, and recheck a new log.
